CICS Student Organization Form

  • This form is being utilized to register your CICS student organization as active for the 2024-2025 Academic Year. Please note that this form does not complete your application for funding. All other requirements in the student organization handbook must be met.
  • The priority funding deadline to complete this form is April 1st of each year by 5 PM EST.
  • Please answer the questions below regarding your organization and its leaders for the upcoming Fall and Spring semesters.
